This card is for unpaid carers over the age of 18 - individuals who are caring for a friend or family member who is unable to cope on their own due to a disability, illness, or substance abuse problems. The carer or person they are caring for must be living in Gwynedd, Anglesey, or Conwy.
Paid care workers are not eligible for this card.
How does the card work?
Your picture and name will appear on the front of the card, along with the date the card expires.
The card will be valid for 5 years.
There is a strip on the back of the card to write down the phone number of the person who has agreed to be an emergency contact.

You can register with Carers Outreach Service to apply for a Carers Card and gain access to additional information and support.
