Carers Card for those over 18 years old

Who is the card for?

This card is for unpaid carers over the age of 18 - individuals who are caring for a friend or family member who is unable to cope on their own due to a disability, illness, or substance abuse problems. The carer or person they are caring for must be living in Gwynedd, Anglesey, or Conwy.

Paid care workers are not eligible for this card.

 

How does the card work?

  • Your picture and name will appear on the front of the card, along with the date the card expires.
  • The card will be valid for 5 years.
  • There is a strip on the back of the card to write down the phone number of the person who has agreed to be an emergency contact.

Applying for a card

You can register with Carers Outreach to apply for a Carers Card and gain access to additional information and support.

Frequently Asked Questions

Yes:

  • You must be an unpaid carer, aged 18 or over, who lives in, or cares for someone in, Gwynedd, Anglesey, or Conwy.
  • You must register with Carers Outreach in order to apply
  • The card is valid for five years, and you will need to apply for a new one before it expires by calling Carers Outreach.
  • If you are no longer a carer, you should contact Carers Outreach, and you will need to return the card within three months.
  • The card cannot be shared with anyone else.
