What help is available to you?
Use the online self-assessment tool, Helpu’n Hun, to find information about products to make daily living activities in your home easier. The Helpu’n Hun assessment offers advice to help you with difficulties you may face with:
Your home – e.g. the bathroom, stairs, lounge, garden.
Everyday activities - e.g. shopping, getting dressed, food preparation.
Your health – e.g. medicine management, home safety, sight and hearing.

You can get help and information to easily access support, activities and services in your community through your local Community Hub.

If you’ve completed the above self-assessment and feel that you need a further discussion, you can contact us.
We will work with you to assess your needs, whilst considering your personal strengths and the support available to you by family members, friends and others in the community.
It’s possible that the assessment will show that certain equipment can help you around the house or as you travel or socialise.
It’s possible to buy equipment privately and to self-refer yourself to some services without receiving an assessment. These are some of the services that are available.
Help around the house
Telecare is a service that allows you to call for help day or night by touching a button or through a series of sensors in your home. It is a preventive way of offering care from a far for residents and it is a way to reduce risk within the home or to allow individuals to continue to live as independently as possible.
If you live in your own property/private rental and need guidance, minor modifications to the home or support with planning and organising major modifications (e.g. a walk-in shower) privately, please contact:
If you are in a position to be arranging and paying for any other major modification (e.g. a flat access shower) it is recommended that you contact your local builder.
Living in a property owned by a Housing Association
If you live in a property owned by a Housing Association, contact them directly for self-referral for minor modifications (e.g. handrails, key safe).
We are unable to provide support letters for benefits, moving property, blue badges etc.
If you need a support letter, contact the professional who is already working with you. Your worker may share a copy of the latest assessment rather than writing a specific support letter.
As we get older, the risk of a serious injury resulting from a fall increases. This can be a hindrance to how someone lives their life.
The Collection Support Service is available to people who cannot take their recycling bins or boxes out to the collection point due to disability or illness.

Help with travel and socialising
The Wales Mobility Driving Assessment Service completes assessments with drivers, passengers and carers who have difficulties driving or transferring in/out of the car due to medical conditions (physical or cognitive).
