We can give you information on the phone, electronically and on printed leaflets, or we can refer you to someone who can help you. Relevant and timely information will help you be able to make the right decisions.

To assist you to live as independently as possible in your own home, we could provide:

  • small equipment
  • telecare equipment (see below)
  • small adaptations, such as a rail or stair rails
  • bigger adaptations such as a stair lifts or ramps.

Telecare or Assistive Technology provides assurance that help is at hand if needed. The service can help to reduce risks in your home and enable you to live an independent life. 

Information about Telecare Gwynedd

After a period of illness or an accident, you could need help to reclaim your independence, support your quality of life and regain confidence. Enablement is a specialist service which helps you regain skills in everyday activities like

  • washing and dressing
  • getting around inside and outside the home
  • preparing meals.

If Enablement doesn’t work, you could be eligible for some personal care:

  • help to get up from bed, go to the toilet, wash, dress or prepare food
  • a shopping service, pension collection, clothes washing and cleaning service (if the assessment shows this is a priority. This type of care usually supplements significant care from a carer.)

An opportunity to socialise, make friends and enjoy activities. Sometimes, depending on need, a meal and some care will be included.

Partners, family and friends offer valuable care for vulnerable adults. It’s important to meet the needs of the carers too, so they can lead their own life and maintain their own health.

Self-sufficient flats for older people with care and support provided as required. These flats enable people to live independently as long as possible – you could avoid having to go to a care home or hospital, or could be discharged early. There are opportunities to socialise if you so wish.

A number of people live together in care homes, and professional staff are present 24 hours a day. There are many care homes in Gwynedd – some run by the Council and others by businesses or not for profit organisations.

Some people stay at a care home for a short period following illness or surgery or to give respite to their carer. Other people move there to live permanently because they cannot continue to live independently at home.

Your rights

The Older People’s Commissioner for Wales has launched a new guide for older people, which provides crucial information about their rights when moving into and living in a care home.

Specialist services are available for people who have mobility, sight, hearing or learning difficulties:

  • help with day to day tasks
  • advice on equipment and adaptations
  • help with personal care
  • assistance and home adaptations to allow disabled people to live in their own homes, with staff offering support as required
  • short or long term placements for those who need more intensive support than that which can be provided at home
  • Blue Badges give additional parking rights to people with serious walking difficulties
  • advice, guidance or support to go out and about within the community.
