Apply for a Carer’s Assessment

If you care for someone who is elderly, ill, or disabled, you may be entitled to a Carer’s Assessment. This assessment helps us understand the support you need to continue your caring role. Whether you provide care for a family member, friend, or neighbour, we can offer services and advice to improve your wellbeing and ensure you’re not facing this alone.

What is a Carer’s Assessment?

A Carer’s Assessment is a discussion between you and a social care professional about your role as a carer. It looks at the impact caring has on your life, your physical and emotional health, and your ability to manage other responsibilities such as work or family life.

You don’t need to live with the person you care for or provide full-time care to be eligible for an assessment. The focus is on your needs as a carer, not the person you look after.

How to Apply

You can apply for a Carer’s Assessment online, by phone, or in person.

  • Online: Complete the Carer’s Assessment Application Form and submit it directly through our website.
  • Phone: Call our Carer Support Team on 01234 567890, and one of our advisors will guide you through the process.
  • In Person: Visit one of our local council offices, where our staff can help you fill out the form.

There is no cost for a Carer’s Assessment, and it’s available to all unpaid carers in our area.

What Happens After You Apply?

Once we receive your application, we will contact you to arrange a convenient time for the assessment. It can take place over the phone, online, or in person—whichever works best for you. During the assessment, we will discuss your role as a carer and identify what kind of support would make a difference.

After the assessment, we may be able to offer services such as:

  • Respite care, to give you a break
  • Equipment to help with the caring role
  • Emotional and wellbeing support
  • Training on how to provide care
  • Information on financial support available

Support for the Person You Care For

If the person you care for hasn’t already had a Needs Assessment, they may be eligible for one too. This assessment looks at their own care needs and whether additional services or support can be provided to them. You can apply for a Needs Assessment at the same time as your Carer’s Assessment if both you and the person you care for agree.
