If you or a loved one is struggling with daily tasks due to a disability, illness, or age, we can help you access equipment and home modifications to make life easier. Our team offers assessments and recommendations to ensure you have the support needed to maintain your independence and safety at home.
What We Offer
We can provide or arrange for a variety of aids and adaptations, including:
- Mobility aids such as walking frames, grab rails, and ramps.
- Bathroom and kitchen modifications to improve accessibility.
- Equipment to assist with getting in and out of bed or chairs.
- Stairlifts and hoists for easier movement around your home.
- Sensory aids for those with hearing or visual impairments.
How to Request Equipment or Modifications
- Self-Assessment: Start by completing our online self-assessment form. This will help us understand your needs and whether a formal assessment is required.
- Professional Assessment: If your needs are more complex, one of our occupational therapists will arrange a home visit to assess your living environment and recommend suitable equipment or modifications.
- Funding Support: Many of these aids and adaptations are available free of charge for those who qualify. For larger home modifications, such as stairlifts or wet rooms, you may be eligible for a Disabled Facilities Grant (DFG). Our team will guide you through the application process if necessary.
Who Can Apply?
This service is available to residents with a long-term disability, chronic illness, or those aged 65 and over who are struggling to live independently. Carers or family members can also request support on behalf of someone.
